To see your final results and NCQA Reviewer comments, log in to the Interactive Survey System and scroll down the “Welcome” page to the “NCQA Surveys and Results” heading and click on your Survey Tool. If the “NCQA Surveys and Results” Section does not appear on your welcome page, please follow the instructions in the FAQ entitled “How to View Results” available on the NCQA Web site at http://www.ncqa.org/tabid/1016/Default.aspx .
Within 30 days you will receive a recognition packet that includes a Certificate of Recognition. If you should have any questions about the status you received or about the enclosed documents, please contact the Physician Practice Connections – Patient Centered Medical Home program at (888) 275-7585. NCQA will also list your practice and its physicians on our Web site with a recognition seal for Physician Practice Connections – Patient Centered Medical Home. Six months prior to the recognition expiration date indicated above, you will receive the information needed to begin renewing your recognition.
